Our Foundational Pillars

  • We all strive to be comfortable in our own homes. Foundations Supportive Care works tirelessly to provide a safe and comfortable space that our residents are proud to call home. Our apartment homes are well appointed and common areas are decorated with the input of the residents themselves. Residents are encouraged to decorate their rooms to their own, unique tastes and preferences. Special attention is paid to food purchased for the home to ensure it is nutritious and healthy, with limited additives and preservatives and based on the preferences of the people residing in the home.

  • We understand you have a choice in which independent living provider you select and we take pride in offering exceptional, person-centered care that exceeds state regulatory guidelines. Foundations Supportive Care follows best practices relating to nutrition, safety, environmental modifications, technology, and offers evidence-based interventions for behavioral, medical, and other person-specific needs. Our employees receive extensive training and ongoing professional development to ensure they are equipped to provide outstanding care to the persons we support.

  • Community integration is more than living in a community setting. Having access to the community does not mean the person is engaged. We envision a world where people with special needs are accepted and are valued contributors to the communities in which they live. Our residents are encouraged to be participatory, engaged citizens in the community. Based on the persons’ interests, we will work with them to identify opportunities where they can make meaningful impacts in their community. This will look different for each unique person in our program but our goal is to assist our residents in finding ways to connect, engage, and contribute to their community.

  • We’re committed to innovation and apply new and emerging trends from various disciplines to provide outstanding care for the persons we support. We use technology to increase safety, transparency, and communication. Every member of our team thinks outside the box to solve problems and find the best way to serve the people in our program.

  • Collaboration is at the center of what we do. Our residents, families, and staff are our greatest resource and work together to provide outstanding services. Our families are engaged, heard, and valued. We operate from a place of understanding and seek honest and open communication with families, CSBs, and other stakeholders.
