At Foundations Supportive Care, we believe all people should live a life of independence and receive support that is specific to their unique needs. We want you to create your own life and become an engaged citizen in the community you live in. One size doesn’t fit all and that’s why we partner with you to identify how we can help you improve your areas of daily living without compromising your independence.
Residents receiving supported living waiver services will have the opportunity to live in well-appointed apartments located in Northern Virginia. We will work collaboratively with our residents to find the perfect home, decor, and support staff. Together we will develop a plan of support that works best for you. Our luxury-style apartments are conveniently located near shopping, restaurants, and public transportation.
Supported Living Services
Foundations Supportive Care offers group-home waiver support services to persons in need of a higher-level of care and supervision. Trained support staff are on-site 24-hours a day, 7-days a week and assist with all your needs. You will receive support so you can participate in fun and engaging community activities and make and maintain friendships. Staff will be there to help you communicate with and positively interact with your roommates.
Staff will provide full support with your finances, help create a healthy menu for your house and help with cooking delicious meals. If needed, your staff will take you to all your medical appointments and assist with medication management, making sure you get your medications accurately and on time. Your personal hygiene will be important to us and your staff will work to ensure that you are well cared for while also ensuring that you are learning skills to better allow you to engage in daily living activities.
