We’re a community-based, supportive living residential provider that offers outstanding, person-centered residential support to adults with intellectual and developmental disabilities. Providing comfortable housing for residents, we offer support in areas of daily living assistance, so they may live an independent, self-fulfilling life. Working collaboratively with you and your circle of support, we will join you as you seek to enhance your life, providing opportunities for skill acquisition and skill building, positive socialization, community access, increased independence, and wellness. We understand that there is not a one size fits all model to community-based supports and that needs may change over time. We offer in-home supportive services to accommodate the evolving needs of the people in our program. Our goal is to support you or your loved one in the most independent setting as possible.

Are you looking for a place to call home but don’t need the 24-hour care of residential group-home living? At Foundations Supportive Care, we believe all people should live a life of independence and receive support that is specific to their unique needs. We want you to create your own life and become an engaged citizen in the community you live in. One size doesn’t fit all and that’s why we partner with you to identify how we can help you improve your areas of daily living without compromising your independence. Based on your unique needs, our engaging team members may assist you with your finances, help create a healthy menu for your house and help with cooking delicious meals. If needed, we will take you to your medical appointments and assist with medication management.
Residents receiving supported living waiver services will have the opportunity to live in well-appointed townhomes and condominiums located in Northern Virginia. We will work collaboratively with our residents to find the perfect home, decor, and support staff. Together we will develop a plan of support that works best for you. Our luxury-style apartments are conveniently located near shopping, restaurants, and public transportation.
Foundations Supportive Care offers supportive in-home waiver support services to persons who already have their own housing but are in need of daily in-home support. Supportive in-home services are ideal for people who are transitioning out of a residential program, into their own home or for those who are residing at home with their families. Supportive in-home services offer incredibly flexible options for those who do not need a home but are still needing assistance in areas of daily living and safety. Our goal is to keep you living independently, in your own home. We will partner with you and your circle of support to identify exactly how we can best serve you and work collaboratively with you to ensure your needs are met and you’re living the life you’ve always wanted.
Families who are not ready for their loved one to move into their own housing have found supportive in-home services to serve as an effective ‘bridge’ to help their loved one prepare for supported independent living.
